As a courtesy to our patients, Northwest Physical Therapy, Inc. files primary and secondary insurance. We are Medicare Certified and a provider for many managed care plans. Northwest Physical Therapy, Inc. is also happy to provide treatment for work related injuries and accident cases.
We are committed to providing the best treatment for our patients, and our charges are within the ranges of what “usual and customary” is for this area. However, many insurance companies have their own interpretation of what this means. You will be responsible for co-pays and deductibles required by your insurance company. There will be a $35.00 fee for all returned checks. It is always advisable to call your insurance company and find out the specific requirements regarding your coverage.
Northwest Physical Therapy also offers a convenient way for our patients to pay their balances through Care Credit.
